    Optimum cork stopper diameter for a proper wine sealing performance when modifying bottleneck diameter: a first approach

    Aim of study: This study present a theoretical model that allow establishing the proper relationship between forces and diameters that take part in sealing for ensuring an adequate closure during storage time, and obtained the optimum stopper diameter for a proper sealing performance when modifying bottleneck diameter.Area of study: The proposed model is of interested to the whole cork value chain from forest owners to natural cork stoppers manufacturers.Material and methods: The optimum cork stopper diameter depends mainly on stopper quality and the compression rate applied in the bottling operation. In this study, we establish the stopper diameter when reducing bottleneck diameter, applying a compression rate of 33% when corking, and for natural cork stoppers which quality allows to recover its initial diameter to 96% after 24 h since compression.Main results: For a bottleneck diameter of 18 mm, the value of the stopper diameter should be at least of 22.3 mm, and for a bottleneck diameter of 17 mm, the value of the stopper diameter should be at least of 20.3 mm.Research highlights: These results try to solve one of the main worries of natural cork stopper manufacturers, which is the scarcity of raw cork suitable for manufacturing them.     Manejo anestésico y quirúrgico del shunt portosistémico en la especie canina

    Los shunts portosistémicos son anomalías vasculares que permiten el paso de la sangreportal a la circulación sistémica sin pasar por los sinusoides hepáticos y el parénquimahepático. Esto causa un aumento de las sustancias tóxicas en la circulación sistémica pudiendoaparecer alteraciones en el sistema nervioso central. Han sido descritos en perros, gatos,humanos y en algunos otros mamíferos como hallazgos accidentales, y se clasifican encongénitos o adquiridos, intra o extrahepáticos y simples o múltiples. Los signos clínicospredominantes son los que acompañan a la encefalopatía hepática, otros signos neurológicos,síntomas gastrointestinales e incluso urinarios.El diagnóstico presuntivo se basa en la historia clínica, examen físico y alteracionesanalíticas, sin embargo, para su diagnóstico definitivo se requiere el uso de técnicas dediagnóstico por imagen. Se describen dos posibilidades de tratamiento para el shuntportosistémico; el tratamiento médico o conservador, cuya finalidad es identificar y corregirlos factores que predisponen a la encefalopatía hepática, y el tratamiento quirúrgico, que tienecomo objetivo la oclusión del vaso anómalo y restaurar la funcionalidad hepática a unosniveles normales. Para ello existen varias técnicas entre las que encontramos: aplicación deconstrictor ameroide, banda de celofán, oclusor hidráulico, PAS-OD, ligadura con sutura deseda y coils trombogénicos. La elección de la técnica depende, entre otros factores, de lalocalización y diámetro del shunt.En el siguiente trabajo se describen las diferentes técnicas quirúrgicas empleadas parala resolución de las comunicaciones vasculares portosistémicas que pueden afectar a laespecie canina, con sus ventajas e inconvenientes. De la misma manera se detallan lasconsideraciones anestésicas en el manejo de esta patología, así como las posiblescomplicaciones asociadas.<br /

    Socioeconomic Status and Distance to Reference Centers for Complex Cancer Diseases: A Source of Health Inequalities? A Population Cohort Study Based on Catalonia (Spain)

    The centralization of complex surgical procedures for cancer in Catalonia may have led to geographical and socioeconomic inequities. In this population-based cohort study, we assessed the impacts of these two factors on 5-year survival and quality of care in patients undergoing surgery for rectal cancer (2011-12) and pancreatic cancer (2012-15) in public centers, adjusting for age, comorbidity, and tumor stage. We used data on the geographical distance between the patients' homes and their reference centers, clinical patient and treatment data, income category, and data from the patients' district hospitals. A composite 'textbook outcome' was created from five subindicators of hospitalization. We included 646 cases of pancreatic cancer (12 centers) and 1416 of rectal cancer (26 centers). Distance had no impact on survival for pancreatic cancer patients and was not related to worse survival in rectal cancer. Compared to patients with medium-high income, the risk of death was higher in low-income patients with pancreatic cancer (hazard ratio (HR) 1.46, 95% confidence interval (CI) 1.15-1.86) and very-low-income patients with rectal cancer (HR 5.14, 95% CI 3.51-7.52). Centralization was not associated with worse health outcomes in geographically dispersed patients, including for survival. However, income level remained a significant determinant of survival

    Segmental neurofibromatosis type 2: discriminating two hit from four hit in a patient presenting multiple schwannomas confined to one limb

    Background: A clinical overlap exists between mosaic Neurofibromatosis Type 2 and sporadic Schwannomatosis conditions. In these cases a molecular analysis of tumors is recommended for a proper genetic diagnostics. This analysis is challenged by the fact that schwannomas in both conditions bear a somatic double inactivation of the NF2 gene. However, SMARCB1-associated schwannomas follow a four-hit, three-step model, in which both alleles of SMARCB1 and NF2 genes are inactivated in the tumor, with one of the steps being always the loss of a big part of chromosome 22 involving both loci. Case presentation: Here we report a 36-year-old woman who only presented multiple subcutaneous schwannomas on her right leg. To help discriminate between both possible diagnoses, an exhaustive molecular genetic and genomic analysis was performed on two schwannomas of the patient, consisting in cDNA and DNA sequencing, MLPA, microsatellite multiplex PCR and SNP-array analyses. The loss of a big part of chromosome 22 (22q12.1q13.33) was identified in both tumors. However, this loss involved the NF2 but not the SMARCB1 locus. SNP-array analysis revealed the presence of the same deletion breakpoint in both schwannomas, indicating that this alteration was actually the first NF2 inactivating hit. In addition, a distinct NF2 point mutation in each tumor was identified, representing independent second hits. In accordance with these results, no deletions or point mutations in the SMARCB1 gene were identified. None of the mutations were present in the blood. Two of the patient's children inherited chromosome 22 deleted in schwannomas of the mother, but in its wild type form. Conclusions: These results conclusively confirm the segmental mosaic NF2 nature of the clinical phenotype presented

    Use of information and communication technologies (ICTs) in cancer multidisciplinary team meetings: an explorative study based on EU healthcare professionals

    Objectives: Multidisciplinary teams in cancer care are increasingly using information and communication technology (ICT), hospital health information system (HIS) functionalities and ICT-driven care components. We aimed to explore the use of these tools in multidisciplinary team meetings (MTMs) and to identify the critical challenges posed by their adoption based on the perspective of professionals representatives from European scientific societies. Design: This qualitative study used discussion of cases and focus group technique to generate data. Thematic analysis was applied. Setting: Healthcare professionals working in a multidisciplinary cancer care environment. Participants: Selection of informants was carried out by European scientific societies in accordance with professionals' degree of experience in adopting the implementation of ICT and from different health systems. Results: Professionals representatives of 9 European scientific societies were involved. Up to 10 ICTs, HIS functionalities and care components are embedded in the informational and decision-making processes along three stages of MTMs. ICTs play a key role in opening MTMs to other institutions (eg, by means of molecular tumour boards) and information types (eg, patient-reported outcome measures), and in contributing to the internal efficiency of teams. While ICTs and care components have their own challenges, the information technology context is characterised by the massive generation of unstructured data, the lack of interoperability between systems from different hospitals and HIS that are conceived to store and classify information rather than to work with it. Conclusions: The emergence of an MTM model that is better integrated in the wider health system context and incorporates inputs from patients and support systems make traditional meetings more dynamic and interconnected. Although these changes signal a second transition in the development process of multidisciplinary teams, they occur in a context marked by clear gaps between the information and management needs of MTMs and the adequacy of current HIS

    La contribución que nos proponemos presentar atiende dos vertientes de la investigación que hemos llevado a cabo en este curso 2014/2015 en la red en investigación en docencia universitaria dedicada a la Acción tutorial de nuestro centro (Filosofía y Letras, UA). En un primer tiempo, daremos cuenta brevemente del análisis de los resultados obtenidos en las ediciones anteriores. Para ello expondremos cuestiones organizativas que han pretendido conseguir un mayor alcance de la labor de los tutores en colaboración con los tutores par, analizaremos y evaluaremos las medidas de mejora implementadas en el transcurso de las ediciones. En un segundo tiempo, analizaremos y evaluaremos el alcance del equipo de la Acción tutorial en su apoyo a los estudiantes en la adquisición de las competencias transversales. Nuestra investigación tiene como objetivo principal la medición de resultados obtenidos desde la implementación de la figura del tutor par en colaboración con el tutor docente así como la ideación de medidas de optimización para asegurar el espacio necesario para el funcionamiento correcto y pleno de la Acción Tutorial en todas sus dimensiones

    La calidad del corcho es una de las cuestiones más relevantes para el sector corchero. Es un proceso complejo que requiere evaluar múltiples factores y manejar simultáneamente variables cuantitativas y cualitativas, en ocasiones, con un alto grado de subjetividad.Se presentan dos objetivos, en primer lugar establecer la relación entre la calidad comercial del corcho y la porosidad medida mediante análisis de imagen en diferentes etapas de la transformación industrial (planchas, láminas y tapones), y en segundo lugar, evaluar el potencial de la tecnología NIRS como método de caracterización de la porosidad y, por tanto, de la calidad industrial. Para ello se ha utilizado un colectivo muestral formado por 479 planchas, 176 láminas y 90 tapones de corcho natural de una pieza, clasificados industrialmente en 4, 2 y 3 clases de calidad, respectivamente. El coeficiente de porosidad se evaluó aplicando técnicas de análisis de imagen, en dos y tres clases de color. Los espectros NIRS se obtuvieron en la sección transversal (planchas y tapones), en la sección tangencial (láminas y tapones) y en la sección radial (tapones), con un espectrofotómetro Foss NIRSystems 6500 SY II, mediante la modalidad de reflectancia remota.El análisis estadístico de la relación entre calidad y coeficiente de porosidad, medido mediante análisis de imagen, permite discriminar dos clases de calidad para planchas y láminas y tres clases para tapones. La clasificación de la imagen en un mayor número de cluster mejora los resultados.Las mejores calibraciones NIRS se obtuvieron para la porosidad medida en 3 clases de color, coincidiendo con los resultados obtenidos mediante análisis de imagen y permiten discriminar dos clases de calidad en planchas (refugo y corcho taponable), las dos clases en láminas (R2=0,83; r2 =0,78) y las tres clases en tapones (R2=0,67; r2=0,53).Debido a las dificultades de automatización del análisis de imagen en las primeras etapas de la transformación industrial, la tecnología NIRS podría ser una herramienta objetiva que permitiera evaluar la porosidad y diferenciar dos clases de calidad en planchas y láminas y tres clases en tapones. AbstractThe quality of cork is one of the most important issues for the cork industry.     Bratislava Statement: consensus recommendations for improving pancreatic cancer care

    Pancreatic cancer is one of the most lethal tumours, and it is the fourth cause of cancer death in Europe. Despite its important public health impact, no effective treatments exist, nor are there high-visibility research efforts to improve care. This alarming situation is emblematic of a larger group of cancer diseases, known as neglected cancers. To address the impact of these diseases, the European Commission-supported Innovative Partnership for Action Against Cancer launched a multi-stakeholder initiative to determine key steps that healthcare systems can rapidly implement to improve their response. A working group comprising 20 representatives from European medical societies, patient associations, cancer plan organisations and other relevant European healthcare stakeholders was organised. A consensus process based on the results of different studies, discussion of research outcomes, and development and endorsement of draft statements resulted in 22 consensus recommendations (the Bratislava Statement). The statement argues that substantial improvements can be achieved in patient outcomes by centralising pancreatic cancer care around state-of-the-art reference centres, staffed by expert multidisciplinary teams capable of providing high-quality care. This organisational model requires a specific care framework encompassing primary, palliative and survivorship care, and a policy environment prioritising the use of quality criteria and performance assessments as well as research investments dedicated to prevention, risk prediction, early detection and diagnosis. In order to address the challenges posed by neglected cancers in general and pancreatic cancer in particular, a specific control strategy tailored to this reality is required
